Below the forum is a questionnaire that I'd like for you to fill out in order to share your life with me to be able to participate in my project of writing a book about chronically ill artists. Please let me know if you'd like to be known as an anonymous participant. I will respect your right to privacy.
Please don't feel that you need to rush this questionnaire. Take as many days as you need to really dig down and honestly answer the questions at length and you need only to answer each question as it applies to your own illness(es)/handicap(s) and your life.
Answer only those questions that pertain to your situation though I believe most of them apply in some way to all of us.

PLEASE NOTE: Currently the questionnaire has developed into four sections. Once you have submitted the part of the form that you have completed please use the BACK BUTTON on your BROWSER to get back to the rest of the form sections. Once you meet a "SUBMIT" button that is the end of that section to the questionnaire. It is made up in parts so that you can continue whenever you wish to.
